Pharmacy franchise in Malaysia 2026: Watsons, Guardian, BIG, Caring and Alpro, checked
There is no Watsons franchise in Malaysia, so there is no Watsons franchise price. Guardian and BIG Pharmacy do not franchise either. Of the large chains we checked, only Alpro publishes a franchise offer, and it does not publish a price. Here is what each brand really offers, what the law requires, and what is still missing.
How this page is kept up to date The text is re-read and its sources re-opened by the editors at least every two months. Last change to this page: 29 October 2026.

The short answer: who sells a pharmacy franchise in Malaysia?
- Watsons: no. Watsons Malaysia runs its own stores. It opened its 888th store, the Watsons Plus flagship at Isetan Lot 10, on 17 October 2026 (Bernama). Its parent, A.S. Watson, signed the first franchise agreement in its history in 2020, for the Gulf states only (CK Hutchison, 8 Sep 2020). So a search for a “Watsons franchise price” in Malaysia has no answer, because there is no franchise to price.
- Guardian: no. Guardian’s stores belong to Guardian Health and Beauty Sdn Bhd, part of DFI Retail Group. The Edge counted 554 stores in May 2025, and about 100 of them had a pharmacist (The Edge).
- BIG Pharmacy and CARiNG: no. Both belong to Big Caring Group, which ran 668 outlets on 30 September 2026. It is not selling franchises (The Star, 14 Oct 2026).
- Alpro: yes, without a published price. Alpro’s “Opportunity Pharmacy Partnership Scheme” (OPPS) is a franchise for pharmacists. It is aimed at independent pharmacies that want to convert and at young pharmacists who want their own outlet (Alpro).
Watsons franchise price in Malaysia: why there is none
Watsons Malaysia runs all of its 888 stores itself (Bernama, 17 Oct 2026). The first franchise agreement in A.S. Watson’s history, signed in 2020, covers the Gulf states only (CK Hutchison, 8 Sep 2020). Any page that quotes a “Watsons franchise price” or “Watsons franchise fee” for Malaysia has no source for it. If you want to run a pharmacy under a brand, Alpro is the large chain that offers a franchise (see below); to open your own, see the rules and our guide to setting up a company in Malaysia.
Is Watsons the same company as Guardian? Is BIG the same as Caring?
Watsons and Guardian are rivals with different owners. Watsons belongs to A.S. Watson, part of Hong Kong-listed CK Hutchison. Guardian belongs to DFI Retail Group, part of the Jardine Matheson group.
BIG and CARiNG are now the same group. BIG Pharmacy bought 75% of Caring from 7-Eleven Malaysia for RM675 million, which valued all of Caring at RM850 million (Malay Mail, 21 Sep 2023). Big Caring Group now runs BIG Pharmacy, CARiNG, Georgetown, Wellings, Ting and iCare pharmacies.

Constant Pharmacy
Constant Pharmacy is the only pharmacy brand for which a Bursa filing documents franchise terms. They date from 2022. Its website no longer resolves, and we found no 2025-2026 source showing the offer is still open, so it is shown for reference and not ranked.
- Franchise offer: documented until 2022; no 2025-2026 source found
- Model: Franchise. In October 2022 Bursa-listed Advance Information Marketing Bhd (AIM), through Advance Medipharm Solutions Sdn Bhd, signed a five-year franchise agreement to run Constant Pharmacy outlets.
- Franchisor: Mediconstant Holding Sdn Bhd (200201018975 / 586638-X)

- Franchise fee (earlier): RM80,000 (Bioalpha franchise model in 2018, plus a 10% management fee on each outlet’s gross profit; about RM500,000 to set up an outlet) (The Edge Financial Daily, 6-12 Aug 2018)
- Royalty: Not published
- Contract term: 5 years (The Edge, 6 Oct 2022)

BIG Pharmacy, CARiNG, Georgetown, Wellings, Ting, iCare (Big Caring Group)
Malaysia’s largest pharmacy chain by its own account. Neither the group’s announcements nor its websites mention a franchise. If you search ‘BIG Pharmacy franchise’, the honest answer is: there is none on offer.
- Franchise offer: No
- Model: No franchise offer found. Outlets are run by the group.
- Parent: Big Caring Group Bhd (1044657-T), Klang. Backed by private equity firm Creador. Founders Lee Meng Chuan and Lim Sin Yin are pharmacists; first outlet in Damansara Uptown in 2006.
- Outlets: 668 (as of 2026-09-30) (The Star, 14 Oct 2026)

Watsons Malaysia
Searches such as ‘Watson franchise Malaysia price’ get no price because there is no franchise. A.S. Watson’s own 2020 statement calls the Gulf deal ‘the first franchise agreement of AS Watson Group in its almost 180 years of history’.
- Franchise offer: No
- Model: Company-run stores. A.S. Watson Group signed its first-ever franchise agreement in September 2020, for the Gulf states with Al-Futtaim. There is no franchise in Malaysia.
- Operator: Watson’s Personal Care Stores Sdn Bhd
- Parent: A.S. Watson Group, part of CK Hutchison Holdings

The market in numbers
- Pharmacies. Malaysia had 4,320 private community pharmacies in 2024, up from 3,513 in 2021 (Ministry of Health, Statistics Report 2024). The ministry has not yet published 2025 figures.
- Pharmacists. 5,902 fully registered pharmacists worked in community pharmacies in 2024, up from 4,281 in 2021 (same report). Every outlet needs one, so recruiting pharmacists is the first constraint on any new pharmacy.
- The largest chain. Big Caring Group reported revenue of RM4 billion for the year to June 2026, up from RM1.1 billion in 2023, and profit after tax of RM231.8 million. It has signed an underwriting agreement for a listing on Bursa Malaysia, offering up to 1.9 billion shares, or 25.5% of its enlarged share capital. The price and listing date had not been announced on 28 October 2026; press reports point to a listing in mid-November (The Star, The Edge, 20 Oct 2026).
The rules in 2026: who can own and run a pharmacy
Pharmacy law
- A pharmacist must be in control. A company may only keep, sell or dispense medicines once the Pharmacy Board has registered it under section 13 of the Registration of Pharmacists Act 1951. Its business must be “under the control and management of a superintendent who is a registered pharmacist”, who cannot act in that role for any other business. Each outlet must be run by the superintendent or by another registered pharmacist (Act 371). A pharmacist named as a non-executive director does not meet this rule.
- Registration and annual certificate. The company registers on the Board’s PRiSMA portal before trading. Registration is free; the annual certificate costs RM200, or RM300 if late (Pharmacy Board guideline).
- Poisons licence. Dispensing needs a Type A licence under the Poisons Act 1952. The state Pharmacy Enforcement office issues it to a pharmacist, for the premises named on it (Ministry of Health). In 2024 the ministry issued 8,024 of them.
- Price display. Since 1 May 2025, the Price Control and Anti-Profiteering (Price Marking for Drug) Order 2025 requires community pharmacies and private clinics to show medicine prices. Companies that do not can be fined up to RM100,000, and the ministry announced full enforcement from 1 January 2026 (The Star, ANN).
- The High Court stay. On 21 May 2026 the High Court stayed enforcement of the order against private medical and dental practitioners, pending its ruling on the Malaysian Medical Association’s judicial review (CodeBlue, 21 May 2026). On 2 August 2026 the MMA said the government had not appealed and that the stay was still in force (MMA, 2 August 2026). We found no judgment on the merits as of 29 October 2026. The ruling and the MMA statement concern doctors and dentists; neither mentions community pharmacies.
- No dispensing separation. Private doctors can still dispense medicines in their own clinics. Pharmacists’ bodies keep asking for change (CodeBlue, May 2025).
- Foreign owners. We found no published rule on foreign ownership of a community pharmacy, so we make no claim either way. The pharmacist requirements above apply to every owner. Take legal advice.
Franchise law
(Legal 500 Malaysia guide, 2025)
- Registration first. A franchisor must register with the Registrar of Franchises before offering a franchise (section 6). A company that does not faces a fine of up to RM250,000 for a first offence. Registration is valid for five years. As a franchisee, you register within 14 days of signing.
- Documents before you sign. The franchisor must give you its disclosure document and the draft agreement at least 10 days before you sign (section 15).
- Cooling-off. The agreement must give you at least 7 working days during which you can cancel it (section 18).
- How to check. Ask any franchisor for its MyFEX registration number before you sign. If it cannot give one, it should not be offering a franchise.
How we score
We only score brands that offer a franchise or partnership to outside investors. The SIM score, out of 10, measures how much you can check before you pay anything. It is not a verdict on the business, and it uses five tests, each worth 2 points:
- Offer documented. The brand publishes a live franchise page (2), or the offer only appears in a filing or press report more than two years old (1).
- Franchise Act registration confirmed. A public source confirms the registration. The MyFEX register could not be reached on 28 October 2026, so no brand scores here yet.
- Entry fee published. The fee appears on the franchisor’s page or in a filing (2), or only in the press (1).
- Network size dated. An outlet count less than 12 months old (2), or an older one (1).
- Accounts public. The franchisor or its parent is listed on a stock exchange.
Alpro scores 3 out of 10: it publishes its offer but no fee, its last outlet count dates from August 2025, and its accounts are not public. Constant Pharmacy’s franchise terms appear in a 2022 Bursa filing: a RM50,000 fee per outlet, RM300,000 of opening stock and a five-year term. We found no sign that the offer is still open in 2026, so we show Constant but do not rank it. Nobody paid to appear on this page, and no payment can change a score.

About this edition
We rebuilt this page from scratch on 28 October 2026. The previous version, published in February, gave franchise fees and royalty rates for Caring and BIG Pharmacy. We could not find those terms anywhere, and neither chain offers a franchise today, so we removed them. We also removed every profit and payback projection. We do not publish forecasts.
Every figure below links to the brand, a Bursa Malaysia filing, the Ministry of Health or the press, with its date. We do not say which franchise to buy. We show what can be checked, score it the same way for everyone, and say what is missing.
What we do not rank
We do not publish projected sales, margins, payback periods or exit valuations. No source supports them for any Malaysian pharmacy franchise, and a projection is not a fact. If a fee is not published by the franchisor, a stock-exchange filing or the press, it is not on this page. Before you sign anything, get the disclosure document, check the registration and have a lawyer read the agreement.
